FOSTER CONTRACT

Fill out this contract after you've completed your foster home training. Thank you for agreeing to become a foster parent for Second Chance. The following outlines your responsibilities as a foster parent. Please read and agree to each item by clicking the box and submitting the form.

Second Chance (“SC”) retains sole and absolute ownership of the animal(s) I am fostering. I further agree to the following:

I agree that SC must authorize any veterinary care or other expenditures for the animal(s). Any costs incurred without SC permission are my financial responsibility and requests for reimbursement will not be honored. SC shall maintain records of all vaccinations for my foster animal(s).

I agree to maintain the proper vaccinations for my own animals while serving as a foster parent. This is a precaution to protect the health and safety of both the foster animals and your own animals. Some animals rescued from organized animal care facilities develop kennel cough (bordetella) or an upper respiratory infection. In addition, foster animals can also have parasites or illnesses such as tick fever, valley fever, parvo or distemper. SC tests all cats upon intake for FIV and feline leukemia. However, SC cannot ensure that all foster cats are FIV and feline leukemia negative. I understand that my own animals can also develop these illnesses if they are not vaccinated and even if they are vaccinated.

I agree that all of my foster animal(s) shall be placed by SC. All persons adopting a SC animal must go through the applicant screening process to assure proper placement in a secure, loving environment.

I understand SC may change foster animal placements at any time for the benefit or the animal and/or the organization. I have no possessory interest in the animal(s).

I understand that I cannot foster more than four(4) animals/units (litter plus mother) at any one time.

I agree to provide water, a temporary home, and SC supplied food and supplies for the animal(s). If I want to provide supplies other than that supplied by SC, I do so at my expense. I agree to comply with all SC policies governing the proper care of animals.

I agree to notify my foster liaison if I begin to foster for another organization at the same time that I am fostering a SC animal. I understand that fostering for another organization will be done at my home’s own risk. SC will not be liable for any expenses related to animals belonging to another organization.

I agree to medicate the animal(s) as prescribed by a veterinarian. I understand that no adult animal can be brought to adoption events while it is on rehabilitating medication, including antibiotics. This does not include maintenance medication such as allergy medication.

I agree to transport the animals(s) to adoption locations as frequently as possible, but at least twice a month. I agree to arrive at the beginning of the adoption event. My foster animal(s) will remain until the end of the event unless prior arrangements with the appropriate SC volunteer have been made in advance.

I agree to give a minimum of 24 hours’ notice if I need an animal removed from my care.

I understand that SC cannot warrant the behavior or temperament of any animal. I further acknowledge that SC cannot be held responsible for any damages caused by the animal to personal or public property while in my care.

I agree to fully and completely release Second Chance, its board, employees, and other volunteers from any liability of any kind arising from any health or temperament issues a Second Chance animal may have or may develop and/or from any and all claims relating to my foster care of a Second Chance animal.

I agree to indemnify and hold harmless Second Chance, its board, employees or other volunteers from any and all losses, claims and damages arising out of or relating to my foster care or possession of a Second Chance animal, including any claims for injury or damage to persons or property caused by the animal.

While in my care, all supplies, food, and medical care costs for the foster animal(s) will be provided by SC. I will work closely, and agree to communicate regularly, with the appropriate SC volunteer during my term as a foster parent.
