DISCLAIMER: Completion of this form does not guarantee placement in the Second Chance program. Completed forms will be considered by the intake committee who will contact you if we are able to accept your cat into Second Chance.

I understand a $25 fee plus $10 for each additional animal will be due at the time of relinquishment. Litters under 3 months old will only be charged one $25 fee.

I acknowledge that there will be a $25 fee at relinquishment subject to the discretion of my assigned intake person.
